The intention of this form is to provide the NOLA Board's project manager with all of the details needed to generate an estimate. Please fill it out as completely as possible. If you have any questions, please email our project manager.

If the counter has a complicated shape, please send blueprints or request measurement services from the project manager and indicate your preference below. PLEASE BE AS DETAILED AS POSSIBLE.

If you have special considerations you would like us to try and make, note them in the "other" section.
